Abstract   Multiple myeloma is a neoplasm of mature and immature plasma cells. Extraosseous tissues frequently are involved in multiple myeloma. The most common sites of involvement are the spleen, the liver, lymph nodes, and the kidneys, but infiltrates may also be encountered in most other tissues, such as thyroid and adrenal glands, ovaries, testes, lungs, pleura, pericardium, skin, and intestinal tract. Intracranial involvement of multiple myeloma is rare and usually presents in one af three forms, cases with single or multiple cranial nerve palsies caused by myeloma deposits in the bone of the base of the skull, cases simulating intracranial space-occupying lesions, and cases with orbital involvement. Evaluation of the extent of disease is essential for proper management of this patient. The treatment and prognosis differs according to whether the extramedullay plasmacytoma is primary or secondary. We report a case of multiple myeloma presented simultaneously with intracranial extramedullary plasmacytoma of pituitary which erodes the sella with compression of optic chiasma.
